Custom field to allow user selection of toolbar position or other CSS

I wasn't aware it's possible to give users control over CSS until:

Could someone please explain how I would go about doing this? for example specifically to have some CSS that would display the textbox toolbar either above or below the textbox based on user selection of a dropdown...
